Case Manager-Vocational

We are currently recruiting for a Vocational Case Manager to work in our Lorain County locations.

Summary:

The Vocational Case Manager provides vocational rehabilitation services and clinical case management to individuals on their assigned caseload through coordination with client, community resources, medical and clinical teams, employer, and others with the ultimate goal being the client’s timely return to gainful employment.

Essential Functions:

Conduct career assessments, identify vocational strengths, analyze vocational and educational records and determine employability field for the candidate. Develop independent vocational cases and devise a suitable long-term vocational plan for the client; coordinate with employers for the return to work candidates. Identify work readiness and transferable skills and advise a suitable profession for the client based on the vocational case file. Assist clients in resolving any mental/physical health issues and work place conflicts to enhance job retention. Motivate clients to pursue their career with dedication and persistence; facilitate job placement and training process.

Identify each client’s barriers to continued employment and help the client in overcoming those barriers; set up long term employment goals for the client and monitor the client in pursuit of those goals. Effectively interact with program participants, coordinators, referral sources, and community resources in order to come up with successful long term vocational rehabilitation programs for the participants.

Ensuring that clients receive all appropriate services. Coordinating and participating in treatment planning and reviews with entire treatment team. Consulting with each client concerning attainment of treatment plan goals and objectives. Conducting needs assessments and preparing service plans. Providing individual and group counseling with respect to case management and vocational life skills. Development of the discharge plan in coordination with other treatment team members. Maintaining clients’ charts in compliance with agency, state, and local guidelines and regulations. Attending and participating in training activities, staff meetings, and supervisory sessions. Participating in performance improvement activities. Other duties as assigned.

General Requirements:

Must adhere to the Ohio Chemical Dependency Professionals Board’s Chemical Dependency Code of Ethics, demonstrate an understanding of addiction as an illness, and have a background/education in case management, social work and/or coursework in chemical dependency. Prior experience working with specified population(s) in the community and/or other settings preferred. Experience in the field of vocational rehabilitation preferred. Must possess excellent written and verbal communication skills.

Experience with Microsoft Office, job jobs/search engines, and interviewing skills required in order to assist clients with resume development, job search, and interview preparedness.

Must have the ability to effectively work with Agency employees, outside contacts, and a diverse client population. Must possess valid Driver’s License and have a good driving record (insurable through agency's automobile insurance carrier.)

Educational Requirements:

Associate Degree or substantial progress towards Associate Degree with emphasis on Addictions Treatment, Rehabilitation Counseling, Social Work, Social Changer or other relevant field preferred.

Certification Requirements:

CDCA required. CRC (Commission on Rehabilitation Counselor) Certification or equivalent preferred.

Amount of Travel: Minimal to moderate, mostly confined to local travel. Use of agency vehicles as needed.

Hours: Full-time, Monday-Friday, 40 hours per week.

Salary Range: Salary commensurate with licensure and experience.
